Reporting To Terah Ryan Key Responsibilities Case Management & Care Coordination:
Manage and coordinate care plans, working closely with healthcare professionals, families, and community organizations. Facilitate participant transitions of care, ensuring compliance with policies and smooth handoffs between care settings. Participant Support & Advocacy Conduct wellness checks to assess the health and well-being of participants in various settings. Assist participants in accessing community resources such as housing, healthcare, and financial assistance.
Screening & Eligibility:
Assist in assessing financial eligibility for programs, including supporting applications and required documentation. Perform screenings to identify participant needs and make appropriate recommendations.
Collaboration & Communication:
Maintain ongoing communication with participants, families, facilities, and external agencies to coordinate care and resolve issues. Skills, Knowledge & Expertise Education & Certification Must possess a Bachelor's degree in Social Work (BSW) from a program accredited by the Council on Social Work Education (CSWE). Active
LLBSW/LBSW
license in the state of Michigan. Minimum of one year of experience working with a frail or elderly population (preferred). Must possess a current, valid driver's license and maintain appropriate auto insurance. Current CPR certification, or willingness to obtain within 6 months of employment with LifeCircles. Skills and Competencies Strong desire to work in a team-based model of care. Excellent verbal and written communication skills. Highly organized with strong time management abilities. Proficient in Microsoft Office Suite (Excel, Outlook, Word) and electronic medical records systems. Demonstrates professionalism, critical thinking, creativity, flexibility, and strong problem-solving skills. Maintains high ethical standards and a strong sense of integrity.
