Collaborate with interdisciplinary teams to develop comprehensive treatment plans that incorporate medical documentation and clinical ethical guidelines. Support patients with specialized needs such as dementia care, autism spectrum disorders, post-acute care requirements, or developmental disabilities. Qualifications Valid LMSW license with eligibility for clinical practice in the relevant state or jurisdiction. Demonstrated experience working with diverse populations across settings such as hospitals, nursing homes, outpatient clinics, or home/community care environments. Proficiency in medical terminology, CPT coding, ICD coding systems (ICD-9/10), and familiarity with Medicare policies. Strong knowledge of social work practice standards, HIPAA compliance, and ethical guidelines specific to hospice & palliative medicine. Experience conducting diagnostic evaluations for mental health conditions including PTSD and depression. Skills in case management, discharge planning, crisis management, and medical social work documentation. Ability to work effectively with individuals from various cultural backgrounds and those with disabilities or developmental challenges. Excellent communication skills for client interaction as well as interprofessional collaboration within healthcare teams.
