A Family, School, or General Case Manager coordinates the delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs. Provide limited counseling to client and their family.

We serve people with Intellectual/Developmental Disabilities (I/DD), Serious Persistent Mental Illness (SPMI), and Serious Emotional Disturbance (SED). What You'll Do Provide case management and support services for children and youth up to age 21. Collaborate with families, schools, and community partners to ensure comprehensive care. Implement person-centered plans and monitor progress toward goals. Maintain accurate documentation and comply with agency standards.
Qualifications Education:
Bachelor's Degree (BA/BS) in Social Work.
Licensure:
Must be licensed in Michigan (LLBSW/LBSW). Must be a Child Mental Health Professional (CMHP) or able to obtain certification. Experience working with children and youth preferred. Skills & Abilities Proficiency in Microsoft Office Suite and standard office technology. Strong communication, organization, and time management skills. Critical thinking and problem-solving abilities. Attention to detail and accuracy in documentation. Certifications Required Emergency Services (E.S.) Certification CAFAS Certification Wraparound Certification Other Requirements Valid Michigan driver's license, vehicle insurance, and good driving record. Ability to work flexible hours and travel within service area. Must pass pre-employment background checks (criminal, driving, education, employment).
