A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Provide individual and group therapy using CBT and psychoeducation. Perform crisis intervention, suicide risk assessments, and patient de-escalation. Coordinate case management, discharge planning, and community referrals. Support patients and families coping with chronic, acute, or terminal illness. Provide counseling, caregiver education, and patient advocacy. Collaborate with physicians, nurses, case managers, and interdisciplinary teams. Identify high-risk patients and develop bio-psycho-social care plans. Coordinate hospice, palliative care, and end-of-life services. Maintain accurate clinical documentation and patient records. Required Qualifications Requirements Bachelor's or Master's Degree in Social Work (BSW/MSW). Active Minnesota LSW, LMSW, or LCSW license. Minimum 2 years of hospital social work experience. Acute care discharge planning experience required. Strong psychosocial assessment and crisis intervention skills.
