401(k) with employer matching to help you build long-term financial stability. Job Summary The Social Worker- MSW delivers clinical social work, psychosocial assessments, and discharge planning services to support patients and families at Moberly Regional Medical Center.
Essential Functions Psychosocial Assessments:
Conduct age-appropriate evaluations (neonatal to geriatric) to identify emotional, social, and financial needs.Patient & Family Support:
Provide clinical counseling, emotional support, and resource education for illness, trauma, loss, or life transitions.Discharge Planning & Care Coordination:
Address psychosocial barriers, coordinate post-acute care (SNF, rehab, home care), and collaborate with healthcare teams for safe transitions.Referrals & Resource Management:
Connect patients to mental health providers, social service agencies, support groups, and financial assistance.Complex Case & Crisis Management:
Provide crisis intervention for high-risk referrals, including abuse/neglect, end-of-life care, adoption, and behavioral health.Quality & Documentation:
Maintain timely, accurate EMR documentation complying with regulatory standards; support readmission reduction and quality initiatives.Team Collaboration:
Work with physicians, nurses, and case managers; serve as a peer resource and mentor. Maintains regular and reliable attendance. Complies with all policies and standards. Performs other duties as assigned. Qualifications Master's Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE) required 2-4 years of clinical social work experience in a healthcare setting preferred Licenses and Certifications LMSW- Licensed Medical Social Worker
- State Licensure required BCLS
- Basic Life Support required Knowledge, Skills, and Abilities Strong knowledge of clinical social work principles, case management, and psychosocial assessments.
