General Description of Duties and Responsibilities:

The Social Worker serves as a member of the social services team performing a variety of direct patient care services, including patient teaching, case management, and referral for additional needed services.

Essential Duties and Responsibilities:

1. Obtaining complete medical and social history from the patient, previous medical record, etc. 2. Formulating, initiating and monitoring patient management plans regarding social service needs for each patient. 3. Assuring all patients are receiving appropriate social service which they are entitled to (Medicaid, WIC, AFDC, etc.) 4. Consulting with other members of the health care team as necessary. 5. Teaching and counseling of patients and families. 6. Maintaining all records to comply with required standards and reviewing records regularly as a part of quality assurance activities. 7. Assisting the Medical Director in a periodic review of the patients' health care records. 8. Performing other necessary duties as required by the Family Health Center, Inc. to meet the goals of providing primary health care services.

Reporting Relationships:

The Social Worker reports to and is supervised by the Director of Social Services.

Evaluation:

The evaluation of work performance will be on-going and will be carried out by the Medical Director. It will include the Essential Duties and Responsibilities of this Position Description, as well as employee attitude and general working behavior. Formal evaluation will be performed annually, but may be initiated at other times by the Director of Social Services or by the Medical Director. At least annually, the evaluation will incorporate relevant findings of the health center's Quality Assessment and Quality Improvement (QA/QI) Program.

Qualifications:

1. Licensed as a Social Worker in the State of Mississippi. 2. Certification by a national credentialing agency. 3. Demonstrated competence to perform the social work aspects of primary health care services. 4.
