Maintain accurate medical documentation and social work records in electronic medical record (EMR) systems while ensuring compliance with HIPAA regulations. Facilitate crisis intervention and manage complex cases involving addiction counseling, PTSD care, dementia or Alzheimer's care, autism spectrum disorders, or developmental disabilities. Support patients through intake processes, mental health counseling, hospice & palliative medicine services, and post-acute care transitions with sensitivity and professionalism. Qualifications Valid state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent certification required. Proven experience in medical social work within inpatient or outpatient settings; hospital experience preferred. Strong knowledge of medical terminology, ICD coding (ICD-10/ICD-9), CPT coding, Medicare regulations, and healthcare documentation standards. Familiarity with EMR systems and experience working with individuals from diverse cultural backgrounds. Skills in crisis management, behavioral health interventions, case management, and client communication are essential. Experience working with geriatric patients, children, or individuals with disabilities is highly desirable. Ability to perform comprehensive patient assessments and collaborate effectively within an interprofessional healthcare team while adhering to HIPAA guidelines.
