This role plays a vital part in providing support and coordination for residents living with dementia, and other memory-related conditions with an emphasis on person-centered care, safety, dignity and quality of life and well-being within a vibrant senior living community. Key Responsibilities Support new resident adjustment and ongoing psychosocial needs Advocate for resident rights while maintaining confidentiality Assess resident needs and assist with transitions between levels of care Provide counseling on advance care planning and aging-related concerns Coordinate referrals and collaborate with interdisciplinary care teams Qualifications Bachelor's or Master's degree in Social Work (BSW or MSW) 1 Year experience minimum working with older adults or in senior living preferred Strong communication, assessment, and problem-solving skills Ability to work independently and collaboratively Long-term Care experience Case Management experience Admissions and discharge experience Detail Oriented Serve as a resident advocate, upholding resident rights Assist Residents and families with Long Term Care Insurance claims.
