Social Worker III APS/Guardianship

Protecting, caring for and empowering children and adults who are temporarily or permanently unable to care for themselves through the administrating of federal, state and county programs focused on improving the well-being of our citizens. The anticipated hiring range for this position is $27.45 to $31.91 per hour. The duties listed below are not all of the duties that may be assigned but are those that are considered as essential for an employee to perform. Coordinate and mobilize Adult Protective Services (APS) and provide ongoing guardianship case management by collaborating with community agencies, service providers, and natural supports; developing and implementing service plans; coordinating services and placements; and making decisions in the best interest of the ward in accordance with court orders to ensure safety, stabilization, and well-being. Assess the need for guardianship by evaluating capacity, supports, and risk factors, and attend court proceedings to provide testimony and support legal determinations regarding incompetency and guardianship. Maintain regular contact through home, hospital, and facility visits to monitor condition, ensure appropriate care, and identify changes requiring intervention. Advocate for clients' and wards' rights and well-being by addressing concerns related to quality of care, exploitation, or unmet needs, and participate in multidisciplinary staffing, case consultations, and emergency or departmental response efforts. Prepare and maintain accurate, timely documentation including case narratives, service plans, court reports, and required guardianship filings to ensure compliance with policy and legal standards. Assist with emergency shelter operations and undertake additional responsibilities as required to support departmental needs. Master's degree in social work from an accredited school of social work Bachelor's degree in social work and one year of experience in the area of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy, or treatment planning Bachelor's degree in a human service field with at least 25 semester hours of approved coursework and two years of experience in a related area Bachelor's degree and three years of related experience. A valid driver's license is required Social Work Assessment and case management skills needed, as well as knowledge of medical and mental health issues/terminologies, and knowledge of geriatrics/aging issues. (b), if the position being filled requires an applicant for employment to work with children in any capacity, the County must require the applicant, if offered the position, to be subject to a criminal history record check conducted by the SBI. These checks must be performed in accordance with G.S.143B-1209.26, which authorizes the SBI to provide fingerprint-based state and national criminal record checks to cities and counties.
