Job Description
Full-Time | 40 Hours per Week | 8-Hour Shifts | 5 Days per Week Position Summary Columbus Community Hospital is seeking a compassionate and dedicated Social Worker to provide educational, psychosocial, and supportive services to patients within the SBH Unit. This position plays a key role in assisting patients and families by coordinating support services, facilitating counseling resources, and promoting positive patient outcomes through effective care coordination and community engagement. Essential Duties and Responsibilities Provide educational, psychosocial, and emotional support to patients and their families. Assess patient needs and develop appropriate plans for support and intervention. Coordinate support services, including counseling referrals and community resources. Collaborate with healthcare providers and interdisciplinary team members to ensure continuity of care. Assist patients in navigating healthcare and community support systems. Advocate for patients and families while promoting access to available resources. Facilitate communication between patients, families, and healthcare professionals. Maintain accurate and timely documentation in accordance with hospital and regulatory requirements. Qualifications Education Master's Degree in Social Work required. Experience Minimum of five (5) years of healthcare social work experience preferred. Licensure/Certification Nebraska State Certification as a Certified Master Social Worker (CMSW) required. Licensed Master Social Worker ( MSW ) and/or Nebraska State Certification as a Licensed Mental Health Practitioner (LIMHP) or DSW as applicable and required by state regulations. Knowledge, Skills, and Abilities Strong counseling and interpersonal communication skills. Ability to coordinate services across multiple disciplines and community organizations. Knowledge of healthcare systems, community resources, and patient advocacy principles. Ability to maintain confidentiality and demonstrate professionalism in all interactions. Excellent organizational and problem-solving skills.
