Bedford, New Hampshire LilyCare Hospice is a growing community-based hospice providing compassionate end-of-life care to patients and families in their homes and facilities across New Hampshire. We are hiring a Hospice Social Worker to join our interdisciplinary team. This role is open to full-time or part-time candidates. About the Role As a Hospice Social Worker, you'll be a core member of the Interdisciplinary Team (IDT), providing psychosocial support to terminally ill patients and their families. You'll help patients and families navigate the emotional, practical, and financial realities of end-of-life care while working closely with nurses, chaplains, aides, and physicians. This is a field-based role visiting patients where they live. Responsibilities Complete psychosocial assessments of patients and families and contribute to the Plan of Care Provide counseling and emotional support to patients, families, and caregivers, including during periods of grief, conflict, or crisis Evaluate insurance coverage and financial needs and connect families with community resources and assistance programs Participate in IDT meetings and keep the team updated on psychosocial changes and care plan adjustments Make referrals to community agencies and coordinate services as needed Support bereavement follow-up services for families after a patient's death Maintain accurate, complete, and timely documentation of all visits and services Assist with training and orientation of volunteers and staff, and support special programs such as support groups as needed Participate in quality improvement activities Qualifications Master's degree in Social Work (MSW) from a CSWE-accredited program preferred; BSW from an accredited program considered with MSW supervision Active New Hampshire social work license or eligibility (LICSW/LCSW preferred) Minimum two years of experience working with a geriatric population Hospice or palliative care experience strongly preferred Strong communication skills and the ability to work effectively with patients, families, and healthcare providers at all levels Comfortable navigating difficult family dynamics and grief reactions with professionalism and empathy Valid driver's license, reliable transportation, and ability to travel to patient homes and facilities throughout the service area Why LilyCare Full-time or part-time schedules available Supportive interdisciplinary team environment Orientation and ongoing training in hospice philosophy and practice Competitive compensation and benefits for eligible employees Meaningful work with a mission-driven, growing organization
