The Children's Pyramid has an immediate opening for a full time Early Intervention Service Coordinator to join our team of caring professionals at our Derry, NH location. Our Family Centered Early Supports and Services (FCESS) program provides developmental services and education to children birth to three years of age and their families . Services are provided mainly in homes or at other community locations in Derry and the surrounding towns. As part of a multi-discipline evaluation team, you'll assist in the creation of an Individual Family Support Plan (IFSP) that is designed to address the unique needs of each child and family. You'll also be part of a team of dedicated providers and service coordinators who work closely together to provide mutual support and mentorship.
Responsibilities and Duties:
Guide families through the FCESS process from evaluation to discharge. Ensure the coordination of all services related to a child's programming are adhered to as outlined in the Individual Family Support (IFSP) plan. Serve as a primary contact for families and assist them in gaining access to community and state resources. Provide in home support and administrative assistance to therapy staff as it relates to the IFSP. Assist children and families through the transition process to their local preschool. Assist with monitoring overall program caseload.
Qualifications and Skills:
Minimum Associate's degree in an early childhood or a case management related field; Bachelor's degree is preferred. Strong organization and time management skills with the ability to meet designated timelines. Experience working independently and as part of a team. Strong verbal and written communication skills. Ability to communicate effectively with a variety of people in various settings. Ability to recognize and adapt to the unique circumstances and needs of others. Valid driver's license, appropriate insurance and access to a reliable vehicle. Must have a clean criminal record and be willing to undergo a criminal record search.
Required Technical Skills:
Proficiency in Microsoft Excel, PowerPoint, Outlook, and SharePoint. Proficiency in Adobe Acrobat and other Adobe applications as needed. Ability to create, edit, and maintain documents, spreadsheets, presentations, and electronic records. Physical Requirements Ability to change positions independently as needed,
