Job Description

Regency Home Health, LLC is seeking a dedicated Case Manager to join our team. The ideal candidate will have a passion for helping individuals in need and possess a diverse skill set in areas such as proofreading, behavioral health, mentoring, and childhood development. The caseload will be @ 15-20 students, Grades 9-12. Hours are 7:10am-2:30pm Responsibilities
  • Provide case management services to individuals with various needs
  • Conduct assessments to determine clients' needs and develop individualized care plans
  • Collaborate with clients, families, and other professionals to ensure comprehensive support
  • Utilize behavior management techniques to assist clients in achieving personal goals
  • Implement classroom management strategies for individuals with special education requirements
  • IEP developement and management
  • Communication with families
  • Data Collection and Progress Monitoring
  • Work with people on the autism spectrum to enhance their quality of life Requirements
  • Behavior management experience and transition planning experience preferred
  • Knowledge of childhood development principles
  • Ability to mentor and guide individuals towards positive outcomes
  • Familiarity with special education practices and curriculum development
  • NH Special Education Certification required
