Based On Experience A beautiful skilled nursing facility located near Madison, NJ is seeking a strong Social Worker with a background in long-term care. This role is responsible for supporting residents and families, coordinating care planning, and ensuring compliance with regulatory requirements.

Social Worker Job Description:

Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living Provide information to the resident, and/or family member, on residents' rights and responsibilities

Qualifications:
