Job Description

The Social Worker is responsible for providing social work services to Adult Medical Day Care participants and their families. The Social Worker assists participants with psychosocial needs, care coordination, community resources, advocacy, and adjustment to medical and social challenges. The Social Worker works closely with the interdisciplinary team to promote the participant's overall well-being, safety, independence, and quality of life.
DUTIES AND RESPONSIBILITIES
: Conduct psychosocial assessments and identify participants' social, emotional, and environmental needs. Participate in the development, implementation, and review of individualized care plans. Provide supportive counseling, emotional support, and crisis intervention when appropriate. Assist participants and families with accessing community resources, benefits, transportation, housing, food assistance, and other social services. Advocate for participants' rights, needs, preferences, and dignity. Maintain accurate and timely social work documentation in the participant's medical record/EMR. Participate in interdisciplinary team meetings and communicate relevant participant concerns to appropriate staff. Assist with new admissions, readmissions, and significant changes in participants' psychosocial status. Monitor and address barriers that may affect a participant's ability to remain safely in the community. Provide referrals to appropriate community agencies and healthcare/social service providers. Assist participants and families with discharge planning and transition of services when applicable. Maintain confidentiality and comply with HIPAA and applicable federal and New Jersey regulations. Assist with identifying and reporting suspected abuse, neglect, exploitation, or other safety concerns in accordance with applicable requirements. Maintain professional relationships with participants, families, caregivers, staff, and outside agencies. Participate in quality improvement, audits, in-service education, and other facility activities as assigned. Perform other duties consistent with the Social Worker role and the needs of the Adult Medical Day Care.
QUALIFICATION REQUIREMENTS
To perform this job successfully, an individual must be able to perform each essential duty proficiently. The requirements listed below are representative of the knowledge, skill and/or ability required. The candidate must have a current Social Work License in good standing in the state of New Jersey. EDUCATION/EXPERIENCE
Baccalaureate degree in social work or sociology or psychology with experience in a licensed health care facility (preferably a geriatric care facility). Master's Degree in Social work
LANGUAGE SKILLS
Ability to read, analyze and interpret common scientific and technical journals, reports and legal documents. Ability to respond to common inquiries or complaints from clients, clients' families, regulatory agencies and the public. Ability to effectively present information to upper level management and public groups.
REASONING ABILITY
Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instruction and deal with several abstract and concrete variables.
CERTIFICATE LICENSES REGISTRATIONS
Certified or licensed by the New Jersey State Board of Social Work Examiners.

Employee must be able to cope with the mental and emotional strain of the position. The noise level of the work environment is usually moderate.

$32.00 per hour Expected hours: 40.0 per week
