A Family, School, or General Case Manager coordinates the delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs. Provide limited counseling to client and their family.

Job Description

This is not your typical case manager position. It is much more. At Family Promise, we meet families where they are; we don't just leave them there. Family Success Strategists, as we call case managers, empower families to understand their strengths and challenges and together explore ways to move forward toward a more secure future. We have the responsibility to think creatively and provide holistic support while making sure families maintain agency and dignity.
Responsibilities:
o Perform initial screening and determine eligibility o Employ critical thinking to empower families toward a path to success o Complete intake o Assist families in identifying their skills, strengths, and interests o Assist families in setting goals/identifying achievable steps o Develop a budget with each family and establish family savings plan o Establish a family savings plan and manage client escrow accounts o Assist in career exploration o Work together with family to Identify skill sets necessary to do these jobs o Review progress/outcomes with Director of Family Services and Executive Director o Facilitate workshops o Perform exit interviews o Keep accurate progress notes o Responsible for HMIS data input and program statistics o Research training programs for jobs that pay a living wage o Attend meetings as determined by the
Director of Family Services Requirements:
Bilingual English/Spanish
REQUIRED
Adult case management experience is essential Bachelor's Degree in related field or 5+ years of case management experience Excellent written and verbal communication skills Knowledge of basic financial literacy concepts Knowledge of social service system Hours vary but include some evening hours Must have reliable car Submit resume and cover letter.
Desired Knowledge of HMIS Pay:
$55,000.00 - $60,000.00 per year
