Caring Hospice Services of Central Jersey is seeking a Part Time Social Worker Choose your work location: Ocean and Monmouth Counties
JOB DESCRIPTION
As Social Worker for Caring Hospice, you will provide quality hospice care, end-of-life, educational and grief support services. We are currently seeking a Social Worker to conduct hospice assessments and patient visits.
JOB REQUIREMENTS
Must be a Licensed Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) in the state of New Jersey. Minimum of 2 years of hospice experience preferred in hospice or oncology. Candidate must have previous experience in conducting comprehensive assessments. Ability to work effectively on an interdisciplinary team. Prior experience making funeral arrangements; patient care planning, accessing community resources and discharge planning. Ability to provide grief counseling/support. Ability to foster open communication with patients, patient's families, facility partners, staff and other healthcare providers Must have reliable transportation, and be able to travel to where the patient resides.
