Social Worker, Occupational Health This role supports patients navigating workplace injury, return to work planning, and connection to community and behavioral health resources.

Position Responsibilities:

Conduct psychosocial assessments for patients seen through occupational health services Support return to work planning in coordination with providers, employers, and case managers Connect patients to community resources, behavioral health services, and financial or social supports as needed Assist with care coordination for complex or high risk occupational injury cases Document assessments and interventions in the EMR in line with clinic and regulatory standards Serve as a liaison between patients, providers, and employer or insurance contacts when psychosocial factors affect recovery or return to work Requirements Master's degree in Social Work (MSW) required; LCSW preferred - LMSW considered.
