Job Description

Summary:
The Social Worker is responsible for providing comprehensive social work services to the geriatric population within the Skilled Nursing Facility, specifically focusing on short-term rehabilitation and long-term care residents. This role facilitates the adjustment of residents and their families to the facility, provides ongoing clinical support, and serves as the primary coordinator for discharge planning and transitions of care.
Experience:
Minimum two years post-graduate experience as a social worker in a similar type of healthcare setting is strongly preferred.
Education:
BNN (Bachelors Degree Non Nursing)
Special Education Requirements:
Bachelor's degree in Social Work, Psychology, Mental Health Counseling, or related field.
Licenses:
Essential Functions:
Provides psychosocial evaluation to meet the patient's medically related social needs, including assessment of the social and emotional factors related to the patient's illness, their need for care, response to treatment, and adjustment to care. Facilitates discharge planning to meet the patient's needs for continued care (e.g., home care, sub-acute rehabilitation, acute care transfer) by identifying psychosocial, environmental, and financial problems. Collaborates with the healthcare team (including RNs and Case Managers) to establish and implement an optimal discharge plan, keeping patient and family informed. Documents appropriately and timely (by end of business day) to reflect continuous progress in the plan of care, ensuring all regulatory components are met. Assesses and facilitates financial needs by utilizing charity funds, assisting with applications (disability papers, etc.), and collaborating with the Business Office and outside agencies. Maintains and implements rules, regulations, policies, and laws, including Joint Commission, that impact the Social Work field.
Other Duties Required:
Other duties as assigned. Travel for this position is sometimes required. Participation in community and employee engagement activities is required. The pay for this position ranges from $28.13-$44.57 per hour. #LI-BB1
