Job Summary:
The Social Worker is responsible for providing a variety of direct and related services to patients, their families, and the professional staff. Incumbent is guided by a thorough knowledge of the psychosocial evaluation process, social work intervention, rules & regulations of the Dept. of Health, Education, Welfare, and the rules & regulations of the hospital.
Responsibilities:
Completes psychosocial evaluations Completes collaborative treatment planning and implements patient centered services, involving family and /or care partners in the treatment plan process Conducts individual, group psychotherapy as prescribed by the treatment plan Participates in departmental meetings Documents according to clinical standards Develops after care plans for patients upon discharge including linkages to other agencies as appropriate Proficiency in implementing Suicide Risk Assessment Proficiency in implementing Telehealth Services Policy in compliance with Federal and NYS regulatory guidelines
Required Skills:
Master of Social Work Valid NYS LCSW or LMSW Minimum 3-5 years of experience in Behavioral Health Outpatient setting Bilingual English/Spanish preferred Must be computer literate, familiar with Microsoft Word & Excel Must be able to problem solve and think critically
