Job Summary :

Halo Network Inc. will provide Community Counseling services to individuals placed at the shelter. Community counseling is an individually designed service intended to assist clients who are experiencing significant problems managing the financial, mental, and emotional responses inherent in adjusting to a significant physical or cognitive disability while living in the community. It is a counseling service provided to clients who are coping with altered abilities and skills. It is expected that counseling will be conducted on a short-term basis. The need for counseling could occur at the homeless shelter to help them transition to permanent housing. While the counseling services are primarily provided in a one-to-one session, there are times when it is appropriate to provide this and other related in group counseling setting.

Minimum Qualifications :

Bachelor's degree in Psychology, Social Work, or Mental Health Counseling required, 2+ years of experience preferred, knowledge of mental health, substance abuse, and medical diagnoses and issues, experience working with a substance abuse population preferred.

Skills & Knowledge :

An understanding of addiction & recovery, mental health, and underserved community. Excellent interpersonal skills ability to multi-task, crisis intervention skills, ability to maintain healthy & professional boundaries, utilization of computers for documentation and communication, ability to be a proactive team player, and able to represent the agency well. Will adopt the Family Development Model and Thinking for a Change CBT Model. Knowledge of Suffolk County resources and experience navigating Social Service Agencies is helpful.

Tasks & Responsibilities :

Complete intake and conduct comprehensive assessments to identify individual clients' skills, interests, and employment barriers. Conduct psychosocial assessments and mental status evaluations. Perform group, individual, and family casework/counseling.  Provide referrals with appropriate follow-up for clients and their families. Maintain accurate, timely, and detailed documentation of the client treatment process. Complete incident reports when appropriate. Participate in case conferences, staff meetings, and provide information on co-occurring disorders. Provide education to clients on mental health related issues. Participate in Performance Measurement/Performance improvement activities as requested. Aid the family in their ability to be self-sufficient by teaching such skills as home maintenance, life management, budgeting, and employment. Facilitate family identification of the stressors overwhelming them from without and within the family system and identify/apply the means to decrease stressors. Facilitate an increase in the parents' ability to develop appropriate limits without the use of corporal punishment. Teach the family to locate and access community resources. Teach the family anger management techniques. Complete client contact, goal setting, and outcome documentation. Facilitate weekly individual and group sessions. Collaborate with the site manager on children's activities and issues. Collaborate with parents and schools to ensure timely enrollment. Facilitate Back to School Supply Program. Attend monthly all-staff meetings and weekly client review. Make referrals to outside community resources to assist the family in overcoming identified barriers to keep children safe in permanent home and promote their wellbeing. Complete any and all other tasks assigned by supervisors/Program Manager. This list is not exhaustive and may be supplemented as necessary. Some overtime may be required during the planning and execution of special events.
