Skilled Nursing Facility located in Flushing, NY is seeking to hire a Full-Time Social Worker with Sub Acute experience to join our dynamic team of professionals. Responsibilities of the Social Worker include, but are not limited to, the following: Completing MDS assessments. Facilitate care plan meetings & discharge planning. implement History and Assessment, Advanced Directives and End of Life Planning. Audit psychology paperwork, hospital discharges, room changes, PASRR, and Advanced Directives. Completing all required documentation in the medical record ( EMR) Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication and Organizational Skills Demonstrates judgment of safety and confidentiality issues Strong verbal and written communication skills Strong customer service focus required Knowledge of the community resources Working Knowledge of Sigmacare Good working knowledge of long term care regulations including documentation requirements • Masters Degree in
