THIS POSITION IS A COMPETITIVE CLASS POSITION. THE APPLICANT WILL BE HIRED PROVISIONALLY PENDING A POSSIBLE FUTURE EXAMINATION.

This is a supervisory position designed to coordinate the social work services of the Nursing Home or Mental Health Department. Provides social work services to service recipients of hospitals, nursing homes or health related facilities; does related work as required. Provides coordination of services by facilitating care planning, advance directive planning, assists with discharge planning, and referrals to external service providers, benefits management and communication with family or personal representatives. The work is performed under the supervision of the Licensed MSW or Department Head. Direct supervision may be exercised over subordinate staff. Does related work as required. Examples of Typical Duties Carries a caseload and does all functions of a Social Work Assistant or Assistant Social Worker; Coordinates caseloads provided by Social Work Assistant or Assistant Social Worker in the department; Participates, if requested, in the organization and on-going functioning of the resident council; facilitates family council meetings. Coordinates psychotherapy services. Participates in leadership meetings and trainings as the advocate for residents and representative for the Social Work department. Full Performance Knowledge, Skills, Abilities and Personal Characteristics Good knowledge of the practices and procedures of social work and of the social and psychological factors related to the disease and disability; ability to supervise subordinate staff; ability to work effectively with medical personnel and other supervisory staff; ability to interpret Federal, State and local laws and regulations; ability to prepare reports and keep accurate records; ability to work with others, physical condition commensurate with the demands of the position. Minimum Qualifications Graduation from a regionally accredited or New York State registered four-year college or university with a bachelor's degree in social work, sociology, psychology, human services or resources and three years full time paid social work experience in a health care facility adhering to the standards acceptable to the New York State Health Department.
