JOB DETAILS POSITION SUMMARY

• LMSW Social Worker

POSITION DUTIES

• Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.

MINIMUM REQUIRED QUALIFICATIONS

• Valid license and current registration as an LMSW or LCSW, issued by the New York State Education Department Master's degree in Social Work from a School of Social Work accredited by the Council on Social Work Education 2 years recent social work experience in an acute hospital setting

LENGTH OF ASSIGNMENT

• 16 weeks

SHIFT / HOURS PER WEEK

• Monday
• Thursday, 9a-3pm. In addition, 1/month support group coverage on 4th Monday from 5:30-7pm. 20hrs/wk. SYSTEMS
• Cerner, Paragon
