Job Description

Psych Float Social Worker II/III (2602250) Psych Float Social Worker II/III At Stony Brook Medicine, our Social Workers are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with both medical and behavioral health diagnosis, substance use disorder, disabilities; victims and violence and other health care related social issues. Most recently, Social Work has been instrumental in responding to both Social Determinants of Health and Health care equity. Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence. Duties of a Social Worker II/III may include the following but are not limited to: Patient/Family Assessments Engage in person centered treatment planning and ongoing counseling with individual and family members with a focus on reducing recidivism. Documentation and referral processes based on clinical need and all regulatory requirements Clinical team case conferencing discussions and collaboration Person Centered and Collaborative Treatment Planning Effectuate discharge planning including insurance verification, linkage to community providers, insurance authorizations and utilization reviews, scheduling transportation and follow up phone calls as needed. Work with community agencies in regard to patient post discharge needs. • Supervision of professional social work students Active involvement in performance improvement activities including data collection and utilization of data for quality improvement Provide coverage in all Behavioral Health and Emergency Department areas Float to all Behavioral Health Areas including Mobile Outreach Team Qualifications Social Worker II
Required:
MSW from an accredited Social Work Education School. NYS LMSW or LCSW. At least three year's post Master's Social Work Minimum 1+ year experience in an Inpatient Acute Care Psychiatric setting/Mobile team/ Psychiatric ED and Psychiatric crisis setting. Understanding and ability to work with patients experiencing substance use disorder Demonstrates excellent written, oral and technical communication skills. Ability to work and coordinate with Psychiatric/Medical Treatment Teams in an integrated health model Ability to identify and work with high-risk behavioral health patients Demonstrates understanding of Psychiatric specific community resources. Demonstrates strong assessment and psycho-social skills. Working knowledge of Long Island and Metropolitan NY community resources. Demonstrates patient first, patient advocacy and service excellence.
Preferred:
Experience in substance abuse/counseling intervention, Psych/Medical integration, and Behavioral Health with both adult and pediatric populations. Experience in a psychiatric emergency room or psychiatric crisis setting. Bi-lingual Social Worker III

Anticipated Pay Range:
The salary range (or hiring range) for this position is $86,162 - $109,275 Base The above salary range represents SBUH's good faith and reasonable estimate of the range of possible compensation at the time of posting. The specific salary offer will be based on the candidate's validated years of comparable experience. Any efforts to inflate or misrepresent experience are grounds for disqualification from the application process or termination of employment if hired.
