Qualifications:
Required:
MSW from an accredited Social Work Education School. NYS License within one year of hire. Experience in Medical Health Care. Excellent written and oral communication skills. Ability to work and coordinate with established Medical Treatment Team. Experience in discharge planning and Long Length of stay barriers and systems. Experience in Assessment and psycho-social skills. Working knowledge of Long Island and Metropolitan NY community resources. Ability to demonstrate patient first, patient advocacy and service excellence. Computer literate. Ability to implement and run medical team rounds. Understanding of community systems, of project management and action planning, of team work and team leader focus and of data collection and statistics. Demonstrates ability to manage large and complex case loads. Understands role of SW and Case Management as it pertains to throughput and Length of Stay.Preferred:
Experience in Multiple Medical Specialties in Social Work Health Care. Experience in Hospital, In-Patient setting, or skilled Nursing Facility. Discharge Planning experience. Diagnosis specific Social Work. Bi-lingual.Please Note:
