As a Social Worker supporting our Epilepsy program, you'll play an integral role in helping patients and families navigate complex medical and psychosocial needs across both inpatient and outpatient settings. This rotating role provides the opportunity to work with both pediatric and adult populations while applying your expertise in crisis intervention, resource navigation, mediation, counseling, and patient empowerment. Psychiatric social work experience is strongly preferred, along with strong communication and relationship-building skills. A caregiver in this position works days| full-time| 8:00am—4:30pm (flexibility). No weekends or holidays. A caregiver who excels in this role will: Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emot ional, financial and environmental problems. Perform psychosocial assessments and provide interventions to assist patients and families (e.g., counseling, e ducation, advocacy, referrals, conflict resolution, crisis intervention, problem solving a nd coordinating family/team meetings ). Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans. Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers. Maintain a network of resources for consultation and connect patients with community resources. Supervise S ocial W ork I nterns and assist with training new caregivers. Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes. Minimum qualifications for the ideal future caregiver include: Master's D egree in S ocial W ork from a CSWE accredited program Current state licensure as a Licensed Social Worker (LSW) State licensure as a L icensed Independent Social Worker (LISW) within five years of hire Two years of experience in a hospital or healthcare setting Preferred qualifications for the ideal future caregiver include: Psychiatric social work experience

Physical Requirements:

Maximum Annual Salary:
