Our Social Worker is responsible for assessing hospice patients and their family to identify the various psycho-social, financial, environmental, and community needs as well as providing education to patients and their families regarding the philosophy of hospice. Furthermore, the Social Worker will play a vital role in patient care planning and collaborating with a variety of community resources and partners to connect our patients and their families with resources to help in identified areas of need. Currently, we have a part-time position available. The position may develop into full-time as available.
Position Requirements:
A graduate of a Master's program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3). Minimum of one year's experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying. Experience in a hospice care preferred. Demonstrates good verbal and written communication, and organization skills. Knowledge and/or experience in patient education. Prefer experience within long-term, home health or hospice setting Possesses and maintains current CPR Certification. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. Skill in the use of EMR (Electronic Medical Record), cell phone, text and email systems or the ability to learn. Willingness to flexible with scheduling.
