Perform psychosocial assessments and provide interventions to assist patients and families. This may include counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings. Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans. Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers. Maintain a network of resources for consultation and connect patients with community resources. Supervise social work interns and assist with training new caregivers. Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.
