Job Details Kettering Health Dayton l Dayton l Full-time l Days Responsibilities & Requirements The Social Worker - Care Management II conducts comprehensive psychosocial assessments, provides counseling and support, and coordinates care plans to address the social, emotional, and resource needs of patients and families. This role collaborates with interdisciplinary teams to facilitate safe transitions of care, advocates for patient needs, and connects patients to community resources. Performs other duties as assigned. All job responsibilities are to be carried out in alignment with Kettering Health's mission, vision, values, and B Safe program. Master's degree in Social Work (MSW) At least 1 year of clinical social work experience in a healthcare setting Current Ohio Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW); must maintain active licensure in accordance with Ohio state law. Basic Life Support (BLS) certification, or ability to obtain within 90 days of hire. Preferred Qualifications Experience in care management, discharge planning, or oncology social work Certification in case management (e.g., Accredited Case Manager [ACM] or Certified Case Manager [CCM])
