Description

Position Summary The Family Preservation Specialist provides intensive, home-based services to children, youth, and families at risk of out-of-home placement or transitioning home from placement. The specialist delivers crisis intervention, family support, case management, and skill-building services to promote child safety, family stability, and successful reunification. Essential Responsibilities

Respond to referrals and make initial family contact within 24 hours.

Provide crisis intervention and support on a flexible schedule

Conduct family assessments, safety planning, and goal development.

Provide parenting education, behavioral interventions, and family-focused support.

Assist families with accessing community resources, including housing, healthcare, education, employment, transportation, and legal services.

Coordinate services with child welfare, juvenile justice, schools, courts, and community providers.

Maintain accurate documentation, case notes, and reports.

Participate in team meetings, case reviews, and court proceedings as required.

Qualifications

Bachelor's degree in Social Work, Human Services, Psychology, Criminal Justice, or a related field.

Experience working with children, youth, and families preferred.

Strong communication, crisis intervention, and organizational skills.

Valid driver's license and reliable transportation.
