Job Description

Job Overview We're seeking a compassionate and dedicated Hospice Social Worker to join our interdisciplinary team. In this role, you'll provide emotional support, resource coordination, and advocacy for patients and families navigating end-of-life care. You'll be a vital part of ensuring dignity, comfort, and peace during life's most tender moments Responsibilities Conduct psychosocial assessments for hospice patients and families Develop individualized care plans in collaboration with the hospice team Provide grief counseling, crisis intervention, and emotional support Assist families with advance directives, financial concerns, and community resources Facilitate family meetings and end-of-life planning discussions Maintain accurate documentation in compliance with Medicare and agency standards Participate in interdisciplinary team meetings and contribute to holistic care planning Qualifications Master's degree in Social Work (MSW) from an accredited program Current state licensure (LMSW or LCSW preferred) Minimum 1 year of experience in hospice, palliative care, or medical social work Strong communication, empathy, and organizational skills Valid driver's license and reliable transportation Why Join Us?
