A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Hours:
Monday- Friday, 8:00am
- 4:30pm
Rotation:
Occasional weekends- General Summary
- Supervises and oversees the work activities of Case Management social work staff and works in a team leadership model to develop and maintain departmental operations that are consistent with Hospital and System community needs. May carry a caseload and provides services to patients and families including counseling, discharge planning and referral to community services.

Essential Functions:
- + Assists in selecting staff, trains/orients personnel, evaluates performance, prepares work schedules and makes recommendations for personnel actions.
Common Expectations:
- + Oversees/Conducts CM SW staffing and assignments, performance evaluations and evaluation and testing of competencies (as applicable).

Minimum Education:
- + Bachelors Degree Required and + Masters Degree Preferred
Work Experience:
- + 2 years Relevant experience. Required + Supervisory experience. Preferred
Licenses:
- + Basic Life Support Upon Hire Required + Licensed Social Worker Upon Hire Preferred
