Our staff is expertly trained in helping our highly diverse residents handle stress and trauma. We work hard to provide a comforting sense of normalcy for those who have been affected by things beyond their control because no one's past should define who they are in the future.
Position Description:
The Family Residential Program (FRP) Case Manager provides direct case management services to families participating in Cumberland Youth & Family Services' Family Residential Program. Using a trauma-informed, strengths-based approach, the Case Manager assists families in identifying goals, accessing resources, overcoming barriers, and developing the skills necessary to achieve long-term stability and self-sufficiency. The FRP Case Manager serves as the primary point of contact for participating families and works collaboratively with residents, agency staff, and community partners to coordinate services, monitor progress, and support successful program completion.
Key Expectations/Responsibilities:
Case Management and Resident Support Conduct resident intake assessments and participate in admissions and orientation processes. Complete comprehensive needs assessments and develop individualized service plans in partnership with residents. Maintain regular contact with residents through scheduled case management meetings and follow-up. Monitor resident progress and update service plans as needed to reflect changing needs and goals. Assist residents in addressing barriers related to employment, education, housing stability, transportation, childcare, financial management, health care, and other identified needs. Provide support, encouragement, advocacy, and accountability to help residents achieve program goals. Assist residents with transition planning and successful program completion. Resource Coordination and Community Partnerships Connect residents with community resources and supportive services. Maintain knowledge of community resources, public benefits, and referral opportunities. Coordinate services with schools, healthcare providers, behavioral health providers, workforce programs, childcare providers, and other community agencies. Advocate for residents when appropriate to help secure necessary services and support. Develop and maintain positive working relationships with community partners. Program Coordination Assist with referral review, admissions, orientation, and discharge processes. Maintain accurate resident files and monitor documentation for compliance with agency requirements. Track resident participation, outcomes, and program progress. Prepare reports and program information as requested by agency leadership. Identify service gaps and communicate resident needs and program recommendations to the Chief Program Officer. Group Facilitation and Resident Engagement Facilitate or assist with resident groups, workshops, and educational activities. Support programming related to parenting skills, life skills, financial literacy, employment readiness, health and wellness, and other topics relevant to family stability. Encourage resident participation in program activities and events. Assist with family engagement activities and special events. Crisis Intervention and Resident Safety Provide crisis intervention and support during resident emergencies and challenging situations. Respond appropriately to resident concerns and behavioral issues using trauma-informed practices. Follow agency protocols regarding incident reporting, mandatory reporting, safety concerns, and emergency situations. Communicate significant resident concerns, incidents, and barriers to agency leadership in a timely manner. Documentation and Compliance Maintain accurate, complete, and timely case records. Complete case notes, assessments, service plans, referrals, discharge summaries, and all required documentation. Ensure documentation complies with agency, licensing, funding, and accreditation requirements. Maintain confidentiality of resident information in accordance with applicable laws and agency policies. Team Participation Participate in staff meetings, case staffing meetings, training opportunities, and agency activities. Collaborate effectively with agency staff to ensure quality services and positive resident outcomes. Support Cumberland Youth & Family Services' mission, values, and commitment to providing trauma-informed care. Perform other duties as assigned.
Qualifications:
Bachelor degree in Social Work, Psychology, Sociology or a closely related human services field. Minimum of 1 year of experience in case management, family services, child welfare, or public housing. Experience working with families impacted by poverty, homelessness, trauma, domestic violence, substance use, child welfare involvement, or other barriers to stability preferred. Strong interpersonal and communication skills. Excellent organizational abilities, time‑management skills, and the capacity to manage multiple priorities Demonstrated ability to work collaboratively while maintaining confidentiality and professional boundaries Patience, empathy, and the ability to engage effectively with adults, children, and young adults
Working Conditions/Physical Requirements:
This role is classified as a Scheduled Remote Work Role and is eligible for up to one scheduled remote workday per week. Lift and carry up to 10 pounds, occasionally up to 20 pounds, infrequently up to 50 pounds Performs tasks that may involve standing or walking 75% of the day, sitting 25% of the day Ability to bend, stoop, reach and grasp as necessary Able to respond to telephones and other auditory stimulations Ability to respond in high stress/emergent situations.
Pay:
$46,000.00 - $52,000.00 per year
