Job Description

Job Summary We are seeking experienced Family Support Coordinators/Case Managers for our client. A leader in the industry with 30+ residential facilities and numerous non-residential programs providing supportive services to developmentally disabled individuals. This rewarding career involves working with intellectually and developmentally challenged adults in their homes and community, participating in daily routines and recreational activities. Duties Work with Individuals, their families, and members of their "circle of support" to ensure a safe and healthy living environment. Provide training and habilitation for Individuals to fulfill their desired outcomes as outlined in their Individual Service Plan. Document and complete all reporting in an accurate and timely manner, and engage in proactive and open communications with key stakeholders regarding the Individuals' needs, progress, and any barriers to their success. Experience High School diploma or GED equivalent and be at least 18 years old. Associate degree in a human services field or three (3) years of relevant experience working with Individuals with intellectual or developmental disabilities. A current driver's license and California automobile insurance coverage for driving your personal vehicle to and from various community locations. Ability to read, write, and speak English and Bilingual in one of the following languages: Spanish, Cantonese, Korean, or Vietnamese.
