The STAR Health Center at SUNY Downstate Health Sciences University is a comprehensive ambulatory care program caring for the most vulnerable populations in Central Brooklyn, including people with HIV and HCV, immigrants, LGBTQIA+ populations, reentrants, aging populations and people with mental health and substance use disorders. We are seeking a half-time Social Worker to work as part of a dynamic multidisciplinary team (www.starprogram.nyc). This is a unique opportunity for a social worker seeking part-time employment with comprehensive benefits. Clinical supervision towards LCSW will be provided.

Minimum Qualifications:

REQUIRED

Masters degree in social work and NYS license required (LCSW, LMSW); 1-2 years' post-license experience conducting screening, assessments, individual and group counseling using evidence-based interventions.

PREFERRED

Experience providing substance use counseling using harm reduction approach. Experience in HIV prevention and treatment. Experience working with LGBTQIA+ populations; bilingual English/Haitian Creole or English/Spanish. Experience documenting in electronic medical record.

Responsibilities:

Conduct psychosocial and behavioral health assessments, using validated screens to assess social determinants of health (SDOH), depression, anxiety, trauma, suicide risk, alcohol and substance misuse and intimate partner violence. Reassess annually, at minimum. Develop and implement individualized care plans, in collaboration with other members of the health care team. Provide individual and group counseling both on-site and remotely, as needed for clients, partners and their families. Provide brief evidence-based individual and/or group mental health interventions that meet the needs of clients (such as cognitive-behavioral therapy (CBT), interpersonal psychotherapy, motivational interviewing, and acceptance and commitment therapy.). Provide a maximum of 12 mental health sessions to indicated and/or interested clients, as program capacity allows. Refer patients for psychiatric clinical care and treatment, such as psychiatric evaluation, comprehensive clinical assessment and diagnosis, medication management, and/or crises or intensive services. Refer patients needing a higher level of substance use treatment to appropriate agencies, when indicated. Provide crisis intervention and serve as mental health lead in the STAR Health Center. Provide client follow-up, care coordination, and referrals to ensure comprehensive care delivery. Participate in huddles and interdisciplinary team meetings. Participate in ongoing quality improvement processes and ensure QA of clinical documentation according to funder standards. Record session notes, care coordination and patient follow-up in the electronic health record. Ensure completion and submission of data forms for entry into database systems.

Hours:

HALF-TIME

(18.75 hours per week); some evening sessions required.

Salary Range:
