This Social Services Coordinator under the direction of the Director of Outreach will work in collaboration with the Social Services team to provide a range of social services to neighbors in our service area. This position is responsible for walking our neighbors through our holistic needs assessment process which is designed to provide immediate assistance-such as food, clothing, and financial support-while also identifying underlying needs that may benefit from long-term solutions. Through compassionate one-on-one conversations, this position will assess each neighbor's situation and connect them to additional internal programs/services and external community resources that promote self-sufficiency. She/he will also assist in the evaluation of these programs on an annual basis. This position will supervise interns and volunteers.
Social Services Coordinator Job Duties:
Completes intake process for neighbors (Charity Tracker ID cards) for all services Interviews and evaluates client needs and eligibility for Immediate needs and other OLMCOS programs/services Develops goals and actions steps, through the holistic needs assessment process, with our neighbors using compassionate one-on-one conversations. Keeps accurate and detailed records and case notes for each neighbor using agency mandated software or database with information entered on a timely basis Conducts thorough and timely follow-up with neighbors to ensure needs are being met, building trust through consistent, reliable communication and follow-through on commitments. Assist neighbors with completion of community resource applications Prepares accurate month reports and actively gathers neighbor success stories to support grant reporting and highlight program impact. Assists neighbors with referrals to external strategic partners. Monitors and updates referral information to ensure accuracy and quality of service. Identifies and implements group curriculum to provide neighbors with opportunities for stability and well-being. Participates in outreach events off site as needed to educate and empower the community. Manages an active caseload of 50 neighbors to assist with identifying goals and actions steps. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; and participating in professional societies. Contributes to team effort by accomplishing related results as needed and attending required meetings and retreats Promotes OLMCOS' mission, vision, and values, treating all people with compassion, dignity and respect. Promotes teamwork and collaboration while navigating neighbors cases Participates and assists with special events and fundraisers as needed Assists/fills-in at any of OLMCOS campuses as needed
Education/Certification:
Bachelor's degree in Social Work, Counseling or similar; previous lived experience will also be considered.

Work Environment:
Work is performed in a fast‐paced and sometimes stressful, professional office environment with noise moderate levels. Position requires team members to effectively use a computer at a workstation for long periods of time and to accommodate potentially frequent interruptions. Must be able to lift 20 - 50 pounds.
SCHEDULE
Business hours are Monday through Friday, from 8:30 a.m. to 4:30p.m with an hour lunch. Our standard work hours may vary based upon position requirements. Additional hours may be required during peak times, which may require an adjusted work schedule, and evening/weekend hours in order to meet deadlines and support community events. All work hours above standard hours must have pre approval of the supervisor. Any changes to regular work hours on a long term basis require the approval of the executive director.
Pay:
$40,000.00 - $50,000.00 per year
