Responsibilities include:
Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department. Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care. Complete assessments and participate in comprehensive care plans. Facilitate residents' safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events. Maintain confidentiality of protected health information, including verbal, written and electronic communications. Perform all other assigned tasks as directed by the Administrator, Director of Nursing, or corporate team.
