Job Description

We're seeking a compassionate, dedicated, and licensed Social Worker to join our interdisciplinary team. The Social Worker is responsible for supporting the emotional, psychosocial, and discharge planning needs of our residents while ensuring compliance with federal, state, and facility regulations. This role works closely with residents, families, physicians, and the care team to promote the highest quality of life for those we serve.
Responsibilities:
Complete psychosocial assessments and develop individualized care plans. Provide counseling and emotional support to residents and their families. Coordinate discharge planning and transitions of care. Assist residents and families with advance directives, community resources, and long-term care planning. Participate in interdisciplinary care plan meetings. Advocate for residents' rights and ensure resident concerns are addressed promptly. Maintain accurate, timely, and compliant documentation. Ensure compliance with CMS, state, and facility regulations. Collaborate with physicians, nursing staff, therapy, and other departments to coordinate resident care. Assist with admissions, transfers, and discharge processes as needed.
Qualifications:
Current Texas Social Worker license required (LBSW, LMSW, or LCSW). Bachelor's or Master's degree in Social Work from an accredited program. Experience in a skilled nursing facility, long-term care, rehabilitation, or healthcare setting preferred. Knowledge of CMS regulations and long-term care standards is preferred. Excellent communication, organizational, and interpersonal skills. Ability to maintain confidentiality and work effectively in a multidisciplinary team.
