Role:
As a Social Worker , you will provide emotional support and counseling to our residents, assisting them in coping with the challenges of aging and addressing their physical, emotional, and social needs. You will also work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring that our residents receive the highest quality care and support. Your expertise in social work principles, theories, and practices will enable you to connect with residents and help them build meaningful relationships with their loved ones.
Responsibilities:
Provide social work services to residents, including counseling, support groups, and individualized care planning Conduct initial and ongoing assessments of resident needs, identifying areas where social work intervention is necessary Develop and implement individualized care plans, ensuring that residents' emotional, social, and physical needs are met Collaborate with other members of the interdisciplinary team to ensure that residents receive comprehensive care and support Provide education and training to residents, their families, and other staff members on social work principles and practices Maintain accurate and detailed records of resident interactions, assessments, and care plans Participate in quality improvement initiatives to continuously improve services and outcomes
