Social Worker Position Summary:

The Social worker will assist in planning, organizing, implementing, evaluating and directing the overall operation of the Social Service Department. The Social Worker will provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psychosocial well-being. At least 2 years experience in a Skilled Nursing Facility or Hospital Licensure /
