This role offers flexible scheduling and the opportunity to make a meaningful difference in the lives of individuals facing serious illness. The ideal candidate will possess a strong background in social work practice standards, hospice care, and working with diverse populations, including geriatric patients, and individuals with disabilities. Your expertise will help facilitate patient-centered care, coordinate community resources, and ensure compliance with healthcare regulations such as HIPAA and Medicare guidelines. Duties Conduct comprehensive patient assessments to identify psychosocial needs, including mental health status, family dynamics, and community resource requirements. Provide individual, family, and group counseling sessions utilizing clinical techniques such as motivational interviewing, cognitive behavioral therapy, and behavioral health interventions. Collaborate with interdisciplinary teams to develop personalized care plans that align with hospice and palliative medicine standards. Coordinate discharge planning and facilitate referrals to community resources such as assisted living facilities, mental health services, addiction counseling programs, and support groups. Document all patient interactions accurately within electronic medical records (EMR) systems while adhering to medical documentation standards and HIPAA regulations. Offer crisis intervention when necessary, managing complex situations involving PTSD care, dementia or Alzheimer's disease progression, or developmental disabilities. Support families through grief counseling and provide education on end-of-life care options while respecting cultural backgrounds and individual preferences. Skills Proven experience in hospice care, medical social work, or palliative medicine with a focus on end-of-life issues. Strong knowledge of medical terminology, ICD coding, Medicare requirements, and clinical ethical guidelines. Skilled in crisis management, trauma-informed care (including PTSD), dementia care, Alzheimer's care, and working with individuals with disabilities or developmental delays. Proficiency in EMR systems for documentation; familiarity with inpatient/outpatient settings is preferred. Excellent client communication skills combined with the ability to collaborate effectively within an interprofessional team. Ability to conduct diagnostic evaluations and case management for diverse populations across community and hospital settings. Experience working with children or seniors in assisted living or home & community care environments is highly desirable. Demonstrated commitment to cultural competence by working effectively with individuals from diverse backgrounds while maintaining confidentiality standards such as HIPAA.
