Job Description
We are seeking a Social Worker with over 2 years of experience for a travel assignment in Long Beach, California.
Responsibilities include:
Providing psychosocial assessments and identifying barriers impacting care transitions. Supporting patient and family meetings related to care planning and transitions. Documenting assessments, plans, and communications in medical records.Key duties:
Work 40 hours weekly with 10-hour shifts. Ensure comprehensive care support and documentation.Qualifications and skills:
