Job Description

Windsor Healthcare Residence is seeking a compassionate and dedicated Part-Time Licensed Social Worker to join our team!
Qualifications:
Licensed Social Worker in good standing, as required by the State. Long-term care experience preferred. Strong interpersonal, organizational, and communication skills. Compassionate, resident-centered approach to care. Ability to work collaboratively with residents, families, and the interdisciplinary care team.
Responsibilities:
Conduct resident social assessments and develop individualized care plans. Provide emotional support and counseling to residents and their families. Participate in interdisciplinary care plan meetings. Coordinate discharge planning and community resources. Assist residents and families with adjustment to the facility and available services.
