Role:

As a Licensed Social Worker, you will work collaboratively with our multidisciplinary team to develop and implement patient-centered care plans, providing emotional support and guidance to patients and their loved ones. You will also be responsible for conducting assessments, identifying patient needs, and developing strategies to address those needs. Licensed Social Worker will work closely with our medical team to provide education and support to patients and families, ensuring a seamless transition from hospital to home.

Responsibilities:

Conduct social work assessments to identify patient needs and develop individualized care plans Provide emotional support and guidance to patients and their loved ones Collaborate with the medical team to develop strategies to address patient needs Develop and implement patient-centered care plans Conduct home visits and provide education and support to patients and families Maintain accurate and timely records of patient interactions and care plans
