This role involves collaborating with clinical interprofessional teams to ensure comprehensive patient care and smooth transitions from hospital to home or other care settings. Experience as a Bereavement Coordinator in hospice is highly valued. Duties Conduct medical social work assessments to evaluate patients' psychosocial needs and develop individualized care plans. Communicate effectively with clients, families, and healthcare providers to facilitate understanding and support throughout the care process. Collaborate with interdisciplinary teams to coordinate clinical interventions, discharge planning, and ongoing support services. Assist patients and families in navigating community resources, including financial assistance, counseling services, and support groups. Manage cases by providing emotional support, counseling, and guidance tailored to the unique needs of each individual. Work closely with geriatric patients to address age-specific challenges and promote quality of life. Support hospice patients and their families through bereavement coordination, offering compassionate guidance during difficult times. Ensure compliance with all regulatory requirements related to social work practice within healthcare settings. Requirements Valid licensure as a Licensed Social Worker (LSW or LCSW) in the relevant state or jurisdiction. Proven experience in medical social work, preferably within hospice or palliative care environments. Strong interpersonal skills with the ability to communicate effectively across diverse cultural backgrounds. Experience working with geriatric populations and managing complex psychosocial issues.
