What We Offer Competitive PRN Compensation Flexible Scheduling Build Your Caseload Around Your Availability Choose the Areas You Want to Cover Mileage Reimbursement Consistent Patient Opportunities One-on-One Patient & Family Support Supportive Clinical Leadership Professional Growth Opportunities What You'll Do Complete psychosocial assessments in patients' homes Identify social, emotional, financial, and environmental barriers affecting care Connect patients and families with community resources and support services Assist with long-term care planning and available financial resources Provide short-term counseling and emotional support Assist patients and caregivers with navigating community programs and services Advocate for patient needs and help address barriers to remaining safely at home Collaborate with nurses, therapists, physicians, and other members of the care team Participate in care planning and discharge planning Complete accurate and timely documentation using WellSky What We're Looking For Master's Degree in Social Work Required Appropriate Virginia Social Work Licensure Required Strong assessment, communication, and interpersonal skills Knowledge of community resources and social service programs Ability to work independently in patients' homes Valid Driver's License and reliable transportation Home Health or Healthcare Social Work Experience Preferred, but Not Required WellSky Experience is a Plus Why Human Touch?
