Summary The Social Worker for Pope's Place works collaboratively with members of an interdisciplinary team in order to assist residents and families with complex psychosocial and complex medical needs. These interventions may include, but are not limited to: admission and discharge planning, address all forms of identified abuse/neglect, facilitate guardianship or adoption, assist with end of life decisions, provide appropriate referral resources, bereavement support, adjustment counseling, and develops and supports resident specific behavior management and behavioral health concerns. The Social Worker leads through the principles of High Reliability to support the mission and a culture of safety. Job Responsibilities/Essential Functions Conducts psychosocial assessments and plans with resident and family/guardian to assist with identified psychosocial needs upon residential and transition admission and quarterly thereafter. Completes resident specific Behavior Intervention Plans for residential and transitional residents; and Children's Stabilization Habilitation and Behavior Intervention Plans for Enhanced Respite Services residents. Provides as needed assessment and supports for waiver respite residents. Completes and documents the social work assessment and plan in the electronic medical record, accurately reflecting the patient's current condition, situational factors, transition of care needs and psychosocial imperatives. Communicates with alternate sites of care and/or community agencies including primary liaison with school district contracts to ensure school enrollment and participation in an effective and timely manner, to best address resident needs Supports, provides input on, and participates in resident life enhancement events, activities, inventories, visitation plans, animal therapy, and Eagles Nest family support apartment. Provides routine caregiver rounds and training on Behavior Intervention and Habilitation Plans. Completes Behavior Management Trainer Training and provides caregiver training on a regularly scheduled basis. Develops and maintains current knowledge of federal and state regulations as they pertain to role and the organization; and community-based resources. Required Qualifications Master's Degree in Social Work Washington State Licensed Independent Clinical Social Worker (LICSW) 2 years of related healthcare experience post Master's Degree 2 years of pediatric healthcare experience post Master's Degree preferred Current Healthcare Provider BLS/CPR Food Handler's Card Must pass a State and Federal Background Check, including fingerprints Valid State Driver's License Must maintain current vehicle insurance Proof of licensing required vaccinations Behavior Management Training within 60 days of hire Behavior Management Trainer Training within 90 days of hire Abuse Prevention Systems Sexual Abuse Awareness Training within 90 days of hire Proficient in MS 365 Work Environment This position is inclusive of all resident care programs of Pope's Place. The position requires flexibility as hours of work are subject to operational needs. The environment may include exposure to communicable diseases, bodily fluids, and other potential hazards. Must wear Personal Protective Equipment (PPE) as required and follow strict guidelines to protect self and residents from infectious disease. Travel Out of town travel is not required for this position.

Pay:

$33.42 - $49.30 per hour Expected hours: 20.0 per week
