The Wisconsin Veterans Home at Chippewa Falls is looking for a Social Worker to join our team in caring for America's heroes. We are a 5-star, deficiency free, homelike care community. The Wisconsin Veterans Home at Chippewa Falls provides compassionate and professional long-term, skilled nursing care to veterans and spouses who cannot be cared for in their homes. As a Social Worker, you will: Provide pre-admission, admission, and counseling services to members. Aid members and their families with personal and environmental difficulties, which may predispose illness or interfere with obtaining maximum benefits from skilled care. Formulate and coordinate interdisciplinary discharge plans for members. Performs clinical evaluations on assigned members. Assesses and develops a plan of care that meets member's needs. Maintains members self-esteem, privacy, and confidentiality of personal information. Provides members with opportunities for independence and choice consistent with their care plan. Completes all documents related to social work. Assists members with problem solving and completing a member satisfaction survey form as appropriate in keeping with facility policy. Serves as a liaison between residents, families, the community, and the surrounding community. Preferred Qualifications (Not Required): A Bachelor's degree in Social Work or a Bachelor's degree in a human services field from an accredited academic institution, including, but not limited to: gerontology, sociology, special education, rehabilitation counseling, psychology. Five (5) years of supervised social work experience in a health care setting, working directly with individuals. Maintain current licensure with state Board of Social Workers as required by current state licensure requirements Benefits of Working at the
