Job Description

Clement Manor is a faith-based independent living, assisted living, CBRF and skilled long-term care community, nationally recognized for providing a superior quality environment for older adults. We have exciting opportunity for a Social Worker to join our team. This individual will manage a caseload of approximately 50 residents in our skilled, memory care and transitional care departments.
Key responsibilities include:
Welcome new residents and family members and orient them to Clement Manor Coordinate assessments and care plans Charting/Documenting appropriate clinical notes Audit clinical records Advocate for residents in the investigation resolution of potential resident rights violations Discharge planning Assist resident with external resources and applications Use verbal and nonverbal communication reflecting our values of respect, collaboration and quality service Ideal candidates will have a Bachelor's degree with a major in social work, psychology, sociology or related field as well as a current social worker license for the state of Wisconsin. Preferred experience includes previous experience in a similar setting, experience with discharge planning and exposure to MDS.
